Responsibilities
- Conduct comprehensive patient assessments via telehealth platforms and maintain accurate electronic health records (EHR).
- Provide high-quality clinical care, education, and support to patients with acute and chronic conditions.
- Collaborate with physicians, pharmacists, and other healthcare professionals to develop and implement individualized care plans.
- Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
- Document all clinical interactions, symptoms, and treatments in compliance with HIPAA regulations and agency policies.
- Respond promptly to patient inquiries and escalations to ensure patient satisfaction and safety.
- Participate in quality improvement initiatives and ongoing professional development training.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the state of Florida or a compact state.
- Minimum of 2 years of nursing experience, preferably in telehealth, critical care, or medical-surgical nursing.
- CPR/BLS certification (ACLS required based on specialty).
- Strong computer skills and proficiency with electronic health record systems (EMR/EHR).
- Excellent communication and interpersonal skills with the ability to build rapport remotely.
- Ability to work independently with minimal supervision and manage time effectively.
