Responsibilities
- Conduct comprehensive patient assessments and triage via telehealth platforms.
- Administer medication management and treatment plans remotely.
- Document patient interactions accurately in Electronic Health Records (EHR).
- Collaborate with physicians and multidisciplinary care teams to ensure continuity of care.
- Provide patient education and counseling regarding health conditions.
- Respond promptly to urgent medical inquiries and escalations.
- Adhere to all state nursing regulations and HIPAA compliance standards.
Qualifications
- Current and active Registered Nurse (RN) license in Minnesota.
- Minimum of 2 years of nursing experience in a clinical setting.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification.
- Proficient in using electronic medical records (EMR/EHR) systems.
- Strong verbal and written communication skills.
- Ability to work independently with high attention to detail.
- Reliable high-speed internet and a private workspace.
