Responsibilities
- Conduct comprehensive virtual patient assessments and triage over secure video conferencing platforms.
- Monitor patient health status, vitals, and medication adherence remotely.
- Collaborate effectively with physicians, advanced practice providers, and the wider care team to develop and adjust patient care plans.
- Document patient interactions, assessments, and interventions accurately and in compliance with HIPAA regulations.
- Educate patients and their families on disease prevention, management, and healthy lifestyle choices.
- Handle emergency situations and escalations according to established telehealth protocols and emergency response procedures.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of Arizona.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 2 years of nursing experience preferred, with specific experience in telehealth or home health a plus.
- Strong computer literacy and proficiency with electronic health record (EHR) systems.
- Excellent communication skills, both verbal and written, with a focus on patient empathy.
- Ability to work independently and manage time effectively in a remote work environment.
