Responsibilities
- Conduct comprehensive patient assessments and triage via secure telehealth platforms.
- Develop and implement individualized patient care plans in collaboration with physicians and care teams.
- Monitor patient health metrics remotely and intervene as necessary.
- Document all clinical interactions, assessments, and interventions accurately and timely.
- Provide patient education and counseling regarding chronic disease management and preventative care.
- Collaborate effectively with multidisciplinary teams to ensure holistic patient outcomes.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the District of Columbia.
- Minimum of 2 years of clinical nursing experience (ICU, Med-Surg, or Telehealth experience preferred).
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong computer skills and proficiency with Electronic Health Records (EHR) systems.
- Excellent communication skills with the ability to build rapport with patients remotely.
