Responsibilities
- Provide telehealth nursing services to patients in Washington DC and surrounding areas
- Conduct patient assessments and develop individualized care plans
- Monitor patient progress and adjust care plans as needed
- Collaborate with healthcare professionals to ensure coordinated care
- Document patient interactions and maintain accurate medical records
- Educate patients and their families on health management and disease prevention
- Respond to patient inquiries and provide timely support
Qualifications
- Active Registered Nurse (RN) license in the District of Columbia
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of 2 years of nursing experience, preferably in telehealth or case management
- Strong communication and interpersonal skills
- Proficiency in electronic health records (EHR) systems
- Ability to work independently and manage time effectively
- Current Basic Life Support (BLS) certification
