Responsibilities
- Conduct comprehensive remote patient assessments and triage via secure telehealth platforms.
- Monitor patient vitals, symptoms, and medication adherence using RPM (Remote Patient Monitoring) technology.
- Administer prescribed medications and treatment plans in compliance with state regulations and facility protocols.
- Document all patient encounters, interventions, and outcomes accurately in the Electronic Health Record (EHR) system.
- Collaborate closely with physicians, nurse practitioners, and care coordinators to ensure seamless patient continuity.
- Provide patient education and emotional support to patients and their families regarding chronic disease management and wellness.
- Identify potential health risks early and escalate concerns to the appropriate medical staff immediately.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of California.
- Minimum of 2 years of clinical nursing experience (Acute Care or Telehealth experience preferred).
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
- Proficient in navigating Electronic Health Records (EHR) systems such as Epic, Cerner, or AthenaHealth.
- Strong computer literacy with reliable high-speed internet and a dedicated home workspace.
- Excellent verbal and written communication skills for remote interaction.
- Ability to remain calm under pressure and make critical decisions in a remote setting.
