Responsibilities
- Conduct comprehensive patient assessments via telehealth platforms and ensure accurate documentation in electronic health records (EHR).
- Provide evidence-based nursing care and medication management according to established protocols and physician orders.
- Collaborate with interdisciplinary healthcare teams, including physicians and pharmacists, to develop optimal patient care plans.
- Monitor patient health status, identify potential complications early, and intervene promptly to prevent adverse events.
- Educate patients and their families on disease prevention, health maintenance, and medication adherence.
- Ensure strict compliance with HIPAA regulations and hospital/clinic policies regarding remote practice.
- Participate in quality improvement initiatives and clinical audits to maintain high standards of care.
Qualifications
- Current, active Registered Nurse (RN) license in the state of Colorado (or compact state eligibility).
- Minimum of 2 years of nursing experience in a clinical setting (Telehealth experience preferred).
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong computer skills and proficiency with EMR/EHR systems (e.g., Epic, Cerner).
- Excellent verbal and written communication skills for remote patient interaction.
- Ability to work independently with minimal supervision in a remote environment.
- Strong critical thinking and problem-solving abilities.
