Responsibilities
- Conduct thorough telephone and digital health assessments to evaluate patient symptoms and medical history.
- Apply critical thinking to prioritize patient needs and recommend appropriate levels of care (e.g., home care, urgent care, ER, physician visit).
- Document all interactions accurately and in compliance with HIPAA regulations and clinical standards.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure continuity of care.
- Provide patient education and resources to promote self-management and preventative health.
- Manage incoming triage queues efficiently to minimize wait times and ensure rapid response.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Kentucky.
- Minimum of 2-3 years of nursing experience, preferably in an urgent care, emergency room, or clinical triage setting.
- Advanced certification in triage (e.g., CEN, CRRN, or Triage Nurse Certification) is highly preferred.
- Strong computer literacy with experience in Electronic Health Records (EHR) systems.
- Excellent verbal and written communication skills with the ability to de-escalate tense situations.
- Ability to work independently while maintaining close communication with the broader care team.
