Responsibilities
- Perform comprehensive phone assessments to evaluate patient symptoms and determine the urgency of medical needs.
- Implement and adhere to established triage protocols and clinical guidelines.
- Document all patient interactions and clinical findings accurately and promptly in electronic health records (EHR).
- Facilitate appropriate care referrals, including scheduling appointments, sending prescriptions, or dispatching emergency services.
- Collaborate with physicians and advanced practice providers to ensure continuity of care.
- Provide health education and self-care instructions to patients and families.
- Identify trends in patient calls to improve service delivery and operational efficiency.
Qualifications
- Current and valid Registered Nurse (RN) license in the State of California.
- Minimum of 2 years of acute care or emergency room nursing experience.
- Advanced certification in triage (e.g., CEN, CCRN, or Triage Nurse Certification) is highly preferred.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification required.
- Strong computer literacy and proficiency with Electronic Health Record (EHR) systems.
- Excellent communication skills with a focus on patient empathy and clarity.
- Ability to work under pressure and make quick, critical decisions.
