Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Provide medication management and chronic disease education remotely
- Coordinate care with multidisciplinary teams through secure digital systems
- Document patient encounters accurately in EMR systems
- Respond to patient inquiries and triage urgent situations
- Participate in weekend on-call rotations as required
Qualifications
- Active Ohio RN license (compact license accepted)
- Minimum 2 years clinical nursing experience
- Telehealth or remote care experience preferred
- BLS certification required
- Proficient with EMR systems and telehealth platforms
- Strong critical thinking and problem-solving skills
- Ability to work independently with minimal supervision
