Responsibilities
- Patient Assessment & Triage: Conduct thorough health assessments and triage incoming patient concerns via telehealth platforms with high accuracy and empathy.
- Clinical Care Management: Monitor patient vitals and health status remotely, ensuring all treatment plans are followed and medication adherence is maintained.
- Documentation: Maintain precise, up-to-date electronic health records (EHR) and clinical notes in compliance with HIPAA regulations.
- Collaboration: Communicate effectively with physicians, case managers, and pharmacists to coordinate comprehensive patient care plans.
- Emergency Response: Utilize telehealth tools to provide immediate guidance and support during acute health events.
- Education: Educate patients and families on disease prevention, medication management, and wellness strategies.
Qualifications
- Licensure: Current, unencumbered Registered Nurse (RN) license in the State of Wisconsin.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 2 years of acute care nursing experience preferred.
- Technical Skills: Proficiency with electronic health records and telehealth software; strong computer literacy.
- Communication: Exceptional verbal and written communication skills with the ability to build rapport with patients remotely.
- Reliability: Demonstrated ability to work weekends (Saturday and Sunday) and holidays as required.
