Job Overview:
We are seeking dedicated Registered Nurses to join our growing Telehealth division. In this role, you will utilize state-of-the-art technology to deliver high-quality clinical assessments and care plans to patients requiring weekend support.
Responsibilities
- Conduct comprehensive patient assessments, triage, and evaluations via secure video and telehealth platforms.
- Develop and implement individualized patient care plans in collaboration with physicians and care teams.
- Manage medication reconciliation and provide education on chronic disease management.
- Document all patient interactions and clinical findings accurately into the Electronic Health Record (EHR) system.
- Respond to patient inquiries and escalations in a timely and empathetic manner.
- Participate in quality improvement initiatives and clinical audits.
Qualifications
- Current and active Registered Nurse (RN) license in the State of Ohio.
- Minimum of 2 years of clinical nursing experience, preferably in telehealth, case management, or critical care.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required.
- Strong proficiency with electronic health record systems (EHR) and telemedicine software.
- Excellent verbal and written communication skills with a focus on patient-centered care.
- Ability to work independently while maintaining strict confidentiality and professional boundaries.
