Responsibilities
- Coordinate patient care plans via telehealth platforms
- Conduct virtual health assessments and medication reminders
- Facilitate communication between patients, families, and clinical teams
- Document care activities accurately in EHR systems

Qualifications
- Active CNA certification in any US state
- 1+ years of clinical experience in long-term care or home health
- Proficiency with telehealth software and EHR systems
- Strong communication and empathetic listening skills

- Valid HIPAA training certification
