Responsibilities
- Conduct virtual health assessments and medication reviews via telehealth platforms
- Coordinate care plans with physicians, nurses, and family members
- Monitor patient vitals and symptoms through remote monitoring tools
- Provide health education and emotional support to patients and families
- Document patient interactions accurately in EHR systems
- Coordinate transportation and community resources for patients

Qualifications
- Active Ohio CNA license in good standing
