Responsibilities
- Coordinate patient care plans via telehealth platforms and electronic health records
- Conduct virtual health assessments and medication reconciliation
- Facilitate communication between patients, families, and clinical teams
- Monitor patient progress and escalate concerns to RNs or physicians
- Document care activities accurately in compliance with HIPAA standards
- Support discharge planning and care transitions for Fresno-area patients
- Participate in virtual team huddles and continuing education
Qualifications
- Active California CNA certification required
- Minimum 1 year acute care or long-term experience
- Proficiency with telehealth software and EHR systems
- Strong communication skills in English and Spanish (preferred)
- Ability to work independently with minimal supervision
- High-speed internet and dedicated home workspace
- Valid CPR certification
- Pass background check and drug screening
