Responsibilities
- Coordinate and manage patient care plans remotely, ensuring all medical needs are met.
- Document patient medical history, treatment progress, and care instructions accurately.
- Communicate effectively with healthcare providers, patients, and families via secure telehealth platforms.
- Monitor patient well-being and escalate urgent medical concerns to on-site teams or physicians.
- Assist with scheduling appointments, medication reminders, and follow-up care coordination.
- Maintain strict compliance with HIPAA regulations and company privacy policies.
- Provide emotional support and health education to patients and their families.
Qualifications
- Certified Nursing Assistant (CNA) license required (active and in good standing).
- Basic Life Support (BLS) and CPR certification.
- Previous experience in case management or remote healthcare coordination is preferred.
- Strong computer literacy, including proficiency with EMR/EHR systems and video conferencing tools.
- Excellent written and verbal communication skills.
- Ability to work independently with minimal supervision in a remote environment.
- Valid driver’s license and reliable internet connection.
