Responsibilities
- Provide virtual patient support and care coordination via telehealth platforms
- Assist with medication reminders and health monitoring remotely
- Document patient information accurately in electronic health records
- Communicate effectively with patients, families, and healthcare teams

Qualifications
- High school diploma or equivalent required
- Active Minnesota CNA certification or willingness to obtain immediately

- Ability to work independently and manage time effectively
- Previous healthcare experience preferred but not required
