Responsibilities
- Provide compassionate direct patient care and assistance with daily living activities (ADLs).
- Maintain accurate and timely electronic health records (EHR) for remote documentation.
- Monitor patient health status and report any changes to the supervising nurse immediately.
- Communicate effectively with patients and their families via secure messaging and video calls.
- Assist with medication reminders and ensure patient safety protocols are followed.
- Collaborate with the interdisciplinary care team to develop patient care plans.
- Complete required documentation and shift reports accurately and on time.
Qualifications
- Current Certified Nursing Assistant (CNA) license in the state of Texas required.
- High school diploma or GED equivalent.
- Strong computer literacy and ability to use telehealth platforms and EHR software.
- Valid driver’s license and reliable transportation (for home visit logistics).
- Excellent interpersonal and verbal communication skills.
- Ability to work flexible shifts, including evenings, weekends, and holidays.
- Basic life support (BLS) certification preferred but not mandatory.
