Responsibilities
- Provide virtual patient education and medication reminders via telehealth platform
- Conduct remote health assessments and document findings in EHR systems
- Coordinate care plans between patients, families, and clinical teams
- Monitor patient vitals and report critical changes to RNs
- Assist with insurance pre-approvals and care coordination tasks

Qualifications
- Active CNA certification in Texas (or compact state)
