Responsibilities
- Conduct virtual health assessments and vital sign monitoring via telehealth platform
- Administer medication reminders and track patient compliance
- Provide emotional support and patient education through digital channels
- Collaborate with RNs and care coordinators on treatment plans
- Maintain accurate electronic health records (EHRs)
- Respond to patient inquiries and triage urgent concerns
- Participate in interdisciplinary care conferences
Qualifications
- Active Ohio CNA license in good standing
- Minimum 1 year clinical experience preferred
- Proficient with telehealth technology and EHR systems
- Exceptional communication and empathy skills
- Ability to work independently with minimal supervision
- Strong problem-solving and crisis management abilities
- High-speed internet and dedicated home workspace
- Ohio residency required for state compliance
