Responsibilities

- Coordinate care plans with RNs and physicians
- Provide medication reminders and health education
- Document patient interactions in EHR systems

- Monitor vital signs through remote monitoring devices
- Collaborate with care teams for complex cases
Qualifications
- Active California CNA certification required
- Minimum 1 year clinical experience
- Telehealth platform proficiency (training provided)
- Strong communication and problem-solving skills
- Ability to work independently in remote setting
- Valid CPR certification
- Reliable internet and quiet workspace
