Responsibilities
- Conduct Remote Patient Monitoring: Utilize specialized software to track vital signs, blood pressure, and glucose levels for patients in Georgia and surrounding areas.
- Patient Education & Support: Provide clear, empathetic guidance to patients and families regarding health management, medication reminders, and daily living skills via video and phone.
- Care Coordination: Communicate critical patient updates and concerns to licensed nurses and physicians in a timely manner to ensure continuity of care.
- Documentation: Accurately record patient data, symptoms, and interventions in secure electronic health records (EHR) systems.
- Crisis Management: Follow strict protocols to escalate medical emergencies or urgent situations to on-site support or emergency services.
- Home Safety Assessment: Provide remote guidance to patients on maintaining a safe and healthy home environment.
Qualifications
- High School Diploma or GED: Required (Bachelor's in Nursing preferred but not mandatory).
- Reliable High-Speed Internet: Essential for remote operations.
- Strong Communication Skills: Ability to explain complex medical information in simple terms.
- Computer Literacy: Comfortable navigating web-based platforms and learning new software quickly.
