Responsibilities
- Provide telehealth patient assessments and medication management support
- Coordinate care plans with on-site nursing staff during night shifts
- Monitor vital signs remotely using digital health platforms
- Deliver patient education and emotional support via virtual channels
- Document care activities accurately in electronic health records
- Respond to emergency protocols via telehealth triage systems
Qualifications
- Active Arizona CNA license in good standing
- 1+ years of clinical experience in hospital or long-term care
- Proficiency with telehealth platforms (Epic, Cerner preferred)
- Strong communication skills for virtual patient interactions
- Ability to work independently during overnight hours
- Basic life support (BLS) certification required
- Experience with remote monitoring technologies preferred
