Responsibilities
- Conduct virtual health assessments and medication reviews via telehealth platform
- Provide remote patient education for chronic disease management
- Coordinate in-home care visits for Sacramento residents (flexible scheduling)
- Document patient interactions in our HIPAA-compliant EMR system
- Collaborate with RNs and physicians on care plans
- Respond to patient inquiries within 2-hour SLA
- Conduct wellness checks via video calls for post-surgery patients
Qualifications
- Active California CNA certification required
- Minimum 1 year clinical experience in home care or assisted living
- Proficient with telehealth platforms and EMR systems
- Valid CPR certification
- Strong communication skills with tech proficiency
- Ability to work independently with minimal supervision
- Flexibility to work evenings/weekends for patient needs
- Reliable transportation for in-home visits (Sacramento area)
