Responsibilities
- Provide virtual patient support via telehealth platforms for medication reminders and symptom monitoring
- Conduct remote wellness assessments and health education sessions
- Collaborate with RNs to create personalized care plans for chronic disease management

Qualifications
- Active CNA certification in good standing with state board

- CPR certification and HIPAA compliance knowledge
