Responsibilities
- Deliver remote patient assessments via telehealth platform, monitoring vital signs and documenting changes in condition
- Coordinate care transitions and discharge planning through virtual patient consultations
- Provide health education and medication reminders to patients via secure messaging systems
- Collaborate with RNs and physicians to update electronic health records (EHR) in real-time
- Manage patient scheduling and follow-up appointments through our digital care management system
- Offer emotional support and resources to patients and families during hospital stays
- Participate in virtual huddles to ensure continuity of care across departments
Qualifications
- Active CNA certification in Washington State
- High school diploma or equivalent required
- Basic computer proficiency with ability to navigate EHR systems
- Strong written communication skills for virtual patient interactions
- Empathetic demeanor and ability to build rapport remotely
- Reliable high-speed internet and dedicated home workspace
- Flexibility to work rotating shifts including evenings and weekends
- Previous hospital or telehealth experience preferred but not required
