Responsibilities
- Provide personalized in-home care including medication management and mobility assistance
- Coordinate care plans via telehealth platforms and virtual family meetings
- Monitor client vitals and health status using remote monitoring systems
- Document care activities in electronic health records (EHR) systems
- Act as liaison between clients, families, and healthcare providers
- Assist with daily living activities and companionship services
- Participate in ongoing virtual training and certification programs
Qualifications
- Minimum 1 year professional caregiving experience
- CPR/First Aid certification (or willingness to obtain)
- Proficiency with telehealth platforms and EHR systems
- Valid California driver's license and reliable vehicle
- Ability to pass comprehensive background check
- Strong communication skills for virtual interactions
