Responsibilities
- Provide personalized in-home care assistance including meal preparation, medication reminders, and mobility support
- Monitor client health status and document care plans using our secure remote portal
- Coordinate with family members and healthcare providers via telehealth platforms
- Transport clients to medical appointments and community activities
- Implement personalized care plans with remote oversight from clinical supervisors
- Maintain accurate electronic health records during remote administrative sessions
Qualifications
- Minimum 1 year professional caregiving experience or CNA/HHA certification
- Valid Arizona driver's license with clean driving record
- Reliable transportation for client visits
- Proficiency with telehealth and documentation software
- CPR/First Aid certification (or willingness to obtain)
- Ability to work independently with remote supervision
- Clear background check and TB test results
