Responsibilities
- Provide personalized in-home care including bathing, dressing, and meal preparation
- Monitor vital signs and report health changes to clinical team
- Assist with mobility transfers and light housekeeping duties
- Coordinate telehealth appointments and document care remotely
- Transport clients to medical appointments using personal vehicle
- Administer prescribed medications under supervision
- Maintain accurate electronic health records from home
- Collaborate with family members and healthcare providers
Qualifications
- Minimum 1 year home health aide experience
- Current Tennessee state HHA certification
- Valid driver's license and reliable vehicle
- Proficient in telehealth platforms and EHR systems
- Flexible availability including weekends and evenings
- Strong communication and documentation skills
- Compassionate approach with geriatric clients
- Pass background check and drug screening
