Responsibilities
- Provide personalized in-home care including bathing, dressing, and mobility assistance
- Monitor vital signs and document client health changes via mobile app
- Administer medications and assist with therapeutic exercises
- Coordinate with virtual care team for seamless client support
- Transport clients to medical appointments using company vehicle
- Prepare nutritious meals and maintain safe home environments
- Complete digital care plans and progress reports remotely
Qualifications
- Active Texas HHA certification or CNA license
- Minimum 1 year home health experience
- CPR/First Aid certification (or willingness to obtain)
- Reliable transportation and clean driving record
- Proficiency with telehealth platforms and mobile apps
- Ability to lift 50+ pounds and stand for extended periods
- Clear background check and TB test results
- Bilingual (English/Spanish) preferred
