Responsibilities
- Conduct remote patient assessments and vital sign monitoring via secure video and phone.
- Assist patients with daily living activities and medication management remotely.
- Document patient interactions and health updates accurately in the EMR system.
- Collaborate with RNs and physicians to update care plans and referrals.
- Maintain strict HIPAA compliance and patient confidentiality.
Qualifications
- Current and valid CNA Certification or Medical Assistant license in California.
- High school diploma or GED required.
- Basic computer proficiency and experience with telehealth platforms.
- Strong communication and interpersonal skills.
- Ability to work independently in a remote setting.
