Your Mission:
As a Remote Nurse/CNA Specialist, you will be the first line of support for our patients, conducting assessments, monitoring health metrics, and coordinating care plans. We need passionate individuals who thrive in a dynamic, fast-paced environment.
Responsibilities
- Conduct Remote Patient Assessments: Perform initial and ongoing health evaluations via video calls, phone, or secure patient portals to monitor patient status.
- Manage Care Coordination: Collaborate with physicians and healthcare teams to develop and adjust patient care plans tailored to individual needs.
- Provide Patient Education: Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Document Care Activities: Maintain accurate and up-to-date electronic health records (EHR) detailing patient interactions, vital signs, and care progress.
- Handle Triage Calls: Assist in prioritizing medical requests and directing patients to appropriate emergency services when necessary.
- Remote Monitoring Support: Assist in the setup and troubleshooting of remote patient monitoring devices to ensure seamless data collection.
- Team Collaboration: Communicate effectively with multidisciplinary teams to ensure holistic patient care.
Qualifications
- Certification: Active RN or CNA license (or equivalent nursing qualification) required.
- Experience: Minimum of 1-2 years of experience in nursing or patient care, preferably in a remote or telehealth setting.
- Communication Skills: Excellent verbal and written communication skills with a focus on patient empathy.
- Technical Proficiency: Comfortable using computer systems, EHR software (e.g., Epic, Cerner), and video conferencing tools.
- Problem-Solving: Ability to think critically and make sound medical decisions in real-time.
