Responsibilities
- Deliver comprehensive telehealth nursing assessments and patient consultations via secure virtual platforms
- Develop and implement personalized care plans for diverse patient populations remotely
- Monitor and analyze patient health data using advanced EHR systems and telemonitoring tools
- Coordinate interdisciplinary care teams and facilitate seamless transitions between virtual and in-person services
- Provide patient education and health coaching through digital channels

Qualifications
- Active RN license in Maryland (or compact state) with minimum 2 years clinical experience
- Bachelor of Science in Nursing (BSN) preferred; ADN candidates with extensive telehealth experience considered
- Proficiency in telehealth platforms, EHR systems, and remote patient monitoring technologies
- Strong assessment skills with ability to evaluate complex cases virtually
- Certification in Telehealth Nursing or willingness to obtain within 90 days
- Exceptional communication skills for effective virtual patient interactions
- Demonstrated ability to work independently with minimal on-site supervision
- Experience with chronic disease management and care coordination preferred
