Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Develop and implement individualized care plans remotely
- Coordinate with multidisciplinary teams for complex cases
- Provide patient education on chronic disease management
- Monitor and document patient progress using EMR systems
- Respond to urgent virtual consultations within 2-hour SLA
- Maintain HIPAA compliance in all digital interactions
Qualifications
- Active RN license in good standing (US state required)
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care certification preferred
- Proficient in EMR systems and telehealth platforms
- Excellent communication and critical thinking skills
- Ability to work independently with minimal supervision
- Valid BLS certification
- Experience with chronic disease management
