Responsibilities
- Provide comprehensive virtual patient assessments via telehealth platform
- Develop and execute personalized care plans for chronic disease management

- Conduct medication reconciliation and patient education remotely

Qualifications
- Active New York State RN license
- Minimum 2 years of clinical nursing experience
