Responsibilities
- Deliver high-quality telehealth consultations and patient assessments
- Manage chronic care cases and treatment plans remotely
- Coordinate care transitions between providers and facilities
- Maintain accurate electronic health records (EHR) documentation
- Provide patient education via virtual platforms
- Collaborate with multidisciplinary care teams

Qualifications
- Active Florida RN license (required)
- Minimum 2 years clinical nursing experience
- Bachelor of Science in Nursing (BSN) preferred
- Telehealth or remote care experience
- Strong computer proficiency with EHR systems
- Exceptional communication and documentation skills
- Ability to work independently in a virtual setting
- Current BLS/CPR certification
