Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via secure video platform
- Develop and implement evidence-based care plans for chronic disease management
- Collaborate with multidisciplinary teams to coordinate patient care and transitions
- Document clinical encounters accurately in EMR systems with 24-hour turnaround
- Educate patients on health management protocols and medication adherence
- Maintain HIPAA compliance and data security protocols at all times
- Participate in peer review and quality improvement initiatives
Qualifications
- Active RN license in Texas (compact license accepted)
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience strongly preferred
- BLS/CPR certification required (ACLS preferred)
- Proficient with EMR systems and telehealth technology
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
