Responsibilities
- Conduct virtual patient assessments via telehealth platform
- Develop and implement personalized care plans
- Coordinate with multidisciplinary care teams
- Provide health education and medication management
- Document patient encounters in EHR system
- Monitor chronic conditions and adjust treatments
- Handle emergency triage protocols
Qualifications
- Active Texas RN license (required)
- BSN or higher (preferred)
- 2+ years clinical nursing experience
