Responsibilities
- Conduct thorough telephone and virtual patient assessments using evidence-based triage protocols
- Provide accurate medical advice and prioritize patient urgency based on clinical criteria
- Collaborate with physicians and care teams to coordinate seamless patient transitions
- Maintain detailed electronic health records while ensuring HIPAA compliance
- Handle high-volume call centers during peak hours with exceptional efficiency
- Delicate patient education on home care, medications, and follow-up protocols
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Illinois with minimum 3 years clinical experience
- BLS certification; ACLS preferred
- Proven expertise in emergency/urgent care triage methodologies
- Exceptional critical thinking and decision-making abilities
- Mastery of telehealth platforms and electronic health records (EHR)
- Outstanding communication skills across diverse socioeconomic backgrounds
- Ability to thrive in fast-paced remote work environment
- Experience with Epic or similar EHR systems required
