Responsibilities
- Provide personalized care including medication management, meal preparation, and mobility assistance
- Monitor vital signs and report health changes to clinical team
- Assist with daily living activities (bathing, dressing, grooming)
- Transport clients to medical appointments and social outings
- Maintain detailed care logs and communicate with family members
- Implement specialized care plans for clients with dementia, Parkinson's, or post-surgery recovery
- Ensure safe home environment through fall prevention and hazard mitigation
Qualifications
- Minimum 3 years professional caregiving experience with verifiable references
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) license required
- Valid Pennsylvania driver's license with clean driving record
- CPR/First Aid certification (or willingness to obtain)
- Ability to lift 50+ pounds and stand/walk for extended periods
- Clear background check and drug screening
- Fluency in English; bilingual (Spanish/Chinese) preferred
- Experience with seniors with complex medical conditions
