Responsibilities
- Provide personalized care for clients with complex medical needs
- Assist with daily living activities (bathing, dressing, mobility)
- Administer prescribed medications and monitor vitals
- Coordinate with healthcare professionals and family members
- Document care activities using electronic health records
- Implement specialized care plans for chronic conditions
- Maintain strict confidentiality and HIPAA compliance
Qualifications
- Minimum 2 years professional home health experience
- Colorado state HHA certification
- CPR and First Aid certification
- Valid driver's license and reliable transportation
- Ability to lift 50+ pounds
- Excellent communication skills
- Experience with Dementia/Alzheimer's care preferred
- Background check clearance required
