Responsibilities
- Provide personalized care to patients including bathing, dressing, and grooming assistance
- Administer prescribed medications and monitor vital signs
- Assist with mobility transfers and light housekeeping duties
- Document patient care progress and report changes to clinical staff
- Transport patients to medical appointments and errands as needed
- Comply with all HIPAA regulations and agency protocols
Qualifications
- Valid Texas HHA certification or CNA license
- Minimum 1 year home health or hospice experience
- CPR/First Aid certification (or willingness to obtain)
- Reliable transportation with valid driver's license
- Ability to pass criminal background check and drug screening
- Strong communication skills and compassion for elderly/disabled
