Your Mission:
As a Remote CNA, you will bridge the gap between clinical care and home health, utilizing technology to monitor patient well-being and coordinate care plans effectively while working from Austin.
Responsibilities
- Monitor patient vitals and health status remotely via secure telehealth platforms.
- Document patient interactions and care plans accurately in electronic health records (EHR).
- Communicate effectively with physicians, nurses, and family members regarding patient progress.
- Provide education and guidance on medication management and daily living activities.
- Collaborate with the interdisciplinary team to adjust care strategies as needed.
Qualifications
- Current and valid State CNA Certification and license (Texas).
- Basic Life Support (BLS) and CPR certification required.
- Proficiency in using computer systems, tablets, and telehealth software.
- Strong communication and interpersonal skills.
- Experience in home health or remote patient monitoring is a plus.
