Responsibilities
- Assess patients via telephone and in-person to determine urgency and appropriate care level
- Collaborate with physicians to develop care plans for acute and chronic conditions
- Maintain accurate electronic health records (EHR) documentation
- Coordinate patient transfers between departments and external facilities
- Educate patients on home care instructions and follow-up protocols
- Participate in quality improvement initiatives and departmental training
Qualifications
- Active RN license in California (CA RN)
- Minimum 2 years emergency department or urgent care experience
- BLS and ACLS certifications required
- Strong assessment skills and triage methodology knowledge
- Excellent communication and decision-making abilities
- Experience with Epic EHR system preferred
- Bachelor of Science in Nursing (BSN) preferred
