Responsibilities
- Perform comprehensive phone and secure message triage assessments for a diverse patient population.
- Utilize clinical protocols and decision trees to determine the urgency of patient needs and direct them to the appropriate care setting.
- Educate patients on self-care management, symptom monitoring, and when to seek immediate emergency care.
- Document all patient interactions, assessments, and care plans accurately and promptly within the Electronic Health Record (EHR).
- Collaborate effectively with physicians, nurse practitioners, and other members of the care team to ensure continuity of care.
- Manage incoming high-volume calls with efficiency, empathy, and a focus on patient satisfaction scores.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Texas.
- BSN preferred; Associate degree in Nursing (ADN) with significant relevant experience considered.
- A minimum of 3 years of clinical nursing experience, preferably in a triage, urgent care, or call center setting.
- Strong proficiency in Electronic Health Records (EHR) systems (e.g., Epic, AthenaHealth).
- Excellent verbal and written communication skills with a demonstrated ability to de-escalate tense situations.
- Ability to work independently while adhering to strict protocols and regulatory guidelines.
