Responsibilities
- Assess patient calls via phone, email, and telemedicine to determine urgency and appropriate care pathways
- Collaborate with physicians and multidisciplinary teams to coordinate patient care and resources
- Document accurate clinical assessments and follow-up actions in electronic health records
- Provide education to patients regarding symptoms, medications, and preventive care
- Maintain strict adherence to HIPAA compliance and confidentiality protocols
- Participate in continuous quality improvement initiatives and training programs
Qualifications
- Active RN license in Hawaii (or compact state eligibility)
- Minimum 2 years clinical experience in emergency or ambulatory care
- Current BLS/CPR certification and ACLS preferred
- Exceptional critical thinking and triage assessment skills
- Strong proficiency with electronic health records (Epic experience preferred)
- Excellent communication skills with ability to de-escalate sensitive situations
- Flexibility to work evenings, weekends, and holidays as needed
