Responsibilities
- Assess patient symptoms via phone/video triage to determine urgency and appropriate care pathways
- Collaborate with physicians and specialists to coordinate patient care plans
- Maintain accurate electronic health records and documentation
- Educate patients on home care instructions and preventive measures
- Handle high-pressure situations with calm professionalism during medical emergencies
- Adhere to HIPAA regulations and hospital protocols
- Participate in continuous quality improvement initiatives
Qualifications
- Active NY RN license with at least 2 years of clinical nursing experience
- BLS certification required; ACLS preferred
- Strong triage assessment skills and critical thinking abilities
- Exceptional communication skills for diverse patient interactions
- Experience with EHR systems (e.g., Epic, Cerner)
- Ability to work flexible shifts including evenings, weekends, and holidays
- Valid NY driver's license for occasional site visits
