Responsibilities
- Conduct comprehensive telephone and digital triage assessments for patients presenting with acute and chronic symptoms.
- Evaluate patient history, current complaints, and vital signs to determine the urgency of care and appropriate treatment pathways.
- Document all interactions and clinical findings accurately and promptly within the Electronic Health Record (EHR) system.
- Provide clear, empathetic health education and self-care instructions to patients and their families.
- Collaborate with physicians, nurse practitioners, and other healthcare providers to coordinate seamless patient care transitions.
- Identify and escalate high-risk cases to ensure patient safety and optimal health outcomes.
Qualifications
- Current and active New Jersey Registered Nurse (RN) license (NJ RN).
- Minimum of 2 years of clinical nursing experience, with at least 1 year in triage or telehealth nursing preferred.
- Certification in BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required.
- Strong proficiency in medical terminology and electronic health record systems.
- Excellent verbal and written communication skills with the ability to de-escalate patient anxiety.
- Demonstrated critical thinking and decision-making skills under pressure.
- Ability to work independently while maintaining strict adherence to protocols and compliance standards.
