About the Role:
In this pivotal role, you will serve as the first point of contact for patients seeking medical advice. Your expertise will ensure patients receive the right level of care at the right time, reducing emergency room visits and improving patient satisfaction.
Responsibilities
- Perform thorough telephone assessments of patient symptoms and medical history.
- Apply clinical reasoning to prioritize patient needs and recommend appropriate action plans.
- Evaluate and triage patients to the appropriate level of care (e.g., Urgent Care, ER, Home Care).
- Document all patient interactions and care plans accurately in the Electronic Health Record (EHR).
- Collaborate with physicians, nurse practitioners, and care managers to ensure continuity of care.
- Provide patient education and self-care instructions for acute and chronic conditions.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of Florida.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 2 years of nursing experience, with at least 1 year in a triage or call center setting preferred.
- Strong knowledge of pathophysiology and pharmacology.
- Excellent verbal and written communication skills.
- Proficient in using EMR/EHR systems.
