Responsibilities
- Perform comprehensive telephone and virtual patient assessments to determine the acuity of symptoms and recommend appropriate levels of care.
- Make critical clinical triage decisions to facilitate timely treatment and prevent unnecessary emergency room visits.
- Educate patients on self-care instructions, medication management, and follow-up appointments to ensure continuity of care.
- Document all interactions accurately and securely in the Electronic Health Record (EHR) system.
- Collaborate with physicians, nurse practitioners, and other healthcare staff to ensure seamless patient care transitions.
- Identify and escalate high-risk patients requiring immediate medical attention.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Texas.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- Minimum of 2-3 years of experience in triage, urgent care, or telehealth nursing.
- Strong clinical assessment skills and the ability to think critically under pressure.
- Excellent verbal and written communication skills with a focus on patient empathy.
- Proficiency with electronic medical records (EMR/EHR) systems and HIPAA compliance.
